Prairie City School-Based Community Clinic
About the Clinic
The Prairie City School-Based Health Center (SBHC), located on school grounds, opened in 2024 through a partnership between Community Counseling Solutions, the Grant County Health Department, and the Prairie City School District, with planning that began in 2018 to improve access to healthcare in rural Grant County. The clinic serves students, school staff, and the broader community by providing convenient, accessible care.
A School-Based Health Center (SBHC) is a primary care clinic located on or near school grounds that provides services similar to those found in a doctor’s office, along with health education, outreach, and referrals. In Oregon, SBHCs have supported youth and families since 1986 through community partnerships, helping reduce barriers to care—especially in rural areas like Prairie City—by offering a comfortable setting, convenient access, and earlier care that supports better health outcomes.
Services
The clinic provides a range of primary care services, including:
- Routine well-care and preventative visits
- Sports physicals
- Immunizations
- Diagnosis and treatment of illness and injury
- Routine lab tests
- Prescription medications
- Health education and wellness support
- Fitness and nutrition education
- Referrals for services not available on-site
Mental health services will be available soon.
If a service is not available at the clinic, staff will assist with referrals to other providers.

No one is denied services due to inability to pay. Staff can also help connect patients to insurance resources if needed.

The clinic is staffed by Community Counseling Solutions and the Grant County Health Department and may include:
- Office Assistant
- Nurse Practitioner
- Certified Medical Assistant (CMA)
- Community Health Worker (CHW)
- Qualified Mental Health Associate (QMHA) or Professional (QMHP)
