Cacoon Program

CHILDREN WITH SPECIAL HEALTH CARE NEEDS

This free program provides support, services, and coordination of care for your child with special health care needs. Our goal is to give your child and others a chance to reach their healthiest and fullest potential.

SUPPORT & SERVICES

A public health nurse will:

  • Help you access services for your child, including medical care

  • Refer you to health insurance programs and information

  • Connect you with other families through parent support organizations

  • Help with concerns such as nutrition, development, and behavior

  • Support you by coordinating community services

  • Help you access financial assistance

  • Arrange limited home visits

Eligibility

A local coordinator will determine your eligibility for additional services, based on your child’s condition and your family’s financial resources.

Eligible children must:

  • Be younger than 20; and
  • Have a condition that prevents them from growing, developing, or playing like other children; or
  • Have a condition that places them at risk for becoming disabled.

All children who are at risk or who have a health or developmental problem are eligible for coordination of care, regardless of income.
