Regional ACT Team – Community Counseling Solutions (CCS)

Serving Umatilla & Morrow Counties
Community Counseling Solutions offers Oregon’s only regional Assertive Community Treatment (ACT) team, dedicated to delivering intensive, community-based mental health services to individuals with serious mental illness (SMI).

About the ACT Team
Our multidisciplinary team consists of nine dedicated professionals:

  • Clinical Supervisor/Team Lead
  • Prescriber
  • Nurse
  • Case Manager
  • Mental Health Counselor
  • Dual-Diagnosis Counselor
  • Two Peer Support Specialists
  • Supported Employment Specialist

Service Delivery
Services are primarily provided in the client’s home or community to reduce barriers and increase accessibility. Each client typically receives 2–4 hours of service weekly from 1–2 staff members, with all team members becoming familiar with each client.

  • ACT Educational/Support Groups
  • 24/7 Crisis Phone Access

Target Population
We serve individuals with serious mental illnesses such as:

  • Schizophrenia
  • Bipolar I Disorder
  • Schizoaffective Disorder
  • PTSD

Ineligible Diagnoses include:

  • Primary Substance Use Disorder
  • Traumatic Brain Injury
  • Intellectual/Developmental Disabilities
  • Borderline Personality Disorder

Research indicates ACT is not effective for these diagnoses.

Admission Criteria
To qualify, individuals must:

  • Have an SMI as their primary diagnosis
  • Show significant functional impairments (e.g., housing instability, unemployment)
  • Have a history of high service needs (e.g., frequent psychiatric hospitalizations or ED visits)
  • Have difficulty engaging in traditional outpatient services

Treatment Goals & Outcomes
Our goal is to support clients in achieving maximum independence and stability in the least restrictive environment possible. This includes:

  • Managing medications and finances
  • Strengthening natural supports (family, friends)
  • Pursuing employment or education
  • Managing emotions and behaviors
  • Assistance with activities of daily living
  • Accessing resources and benefits

When ready, clients may graduate to outpatient care with continued support.

Referrals
Referrals are accepted from anyone—CCS staff, community members, friends, or family.

Required Attachments:

  • Most recent mental health assessment
  • Service plan (if available)

Note: Referrals may take 2–3 weeks to process. Clients may be placed on a waiting list if accepted.
