River’s Edge Acute Center for Healing (REACH), owned and operated by Community Counseling Solutions (CCS), is Oregon’s newest and only dual license residential and acute inpatient treatment facility located in Hermiston, OR.

Facility
REACH serves up to six long-term residential clients and up to nine acute care clients at a time. The long-term beds, referred to as Psychiatric Residential Treatment Services (PRTS) are considered Class 1 secure. The acute beds serve our communities when an individual is in crisis and require immediate mental health stabilization. REACH does not serve substance use disorder populations.

Our facility includes a main milieu and dining area where clients can relax, socialize, take their meals with the staff, and engage in QMHA or therapist activities.

A group room is also available for holding skills training and coping skills activities. Groups occur four times per day, daily.

A separate reflection room allows clients experiencing anxiety or other symptoms a safe space to rest.

Each client has the benefit of their own room with ensuite half bath.

There is also an outdoor courtyard area for clients to take advantage of fair-weather days in Hermiston.

Services
REACH provides an all-encompassing mental health stratagem that includes the client, a provider licensed and nationally board certified to provide psychiatric care for clients with mental illness, and a treatment team dedicated to developing a personalized plan of care.

The REACH treatment team is comprised of a Nurse who is responsible to oversee and direct all medical and nursing procedures developed during a client stay, ensuring there are personalized plans of care and the overall therapeutic environment for all clients.

The in-house clinician is a Masters’ level Qualified Mental Health Professional (QMHP) who serves as the main contact for clients to field questions and concerns about personalized plans of care. The QMHP facilitates groups and will provide individual therapy as outlined in client’s personalized plan of care.

REACH staff include Qualified Mental Health Associates (QMHAs) who help with groups as well as aiding the QMHP with meeting treatment goals. A Care Coordinator, also a QMHA, will assist with admittance and discharge planning along with case management.

Residential Associates (RAs) assist clients with navigating many of the daily living activities to ensure clients meet their overall treatment goals.

A Facility Administrator and Assistant Administrator share oversight of the daily management of keeping the facility running smoothly. Both the Administrator and Assistant work with scheduling and overall management of the facility, programs, and staff.

REACH maintains full-time certified cooks who provide nutritious meals prepared daily. Specialized diet needs bare considered, as well as making sure snacks are available outside of mealtimes.

REACH also has trained Maintenance Technicians that handle preventative maintenance and daily repairs.
